New Directions is a non-profit social service agency offering a wide range of resources and services that provide supports in a variety of settings and foster people’s hopes and dreams in their communities.
Adult Home Share (AHS) provides supports to people who may have intellectual disabilities and/or mental health issues. AHS connects the individual with a Home Share Provider, and they live together in the provider’s home. Support offered to the individual is unique and individualized based on their goals, needs and wants.
Responsibilities of the Case Manager include (but are not limited to):
- Working with individuals and their networks of support to coordinate services, including planning, finding resources, making referrals and advocating for services
- Providing guidance to home share providers and support workers to develop and coordinate supports for the individuals receiving services
- Working with collateral services
- Participating in the On Call Rotation
Successful applicants will have:
- B.S.W./B.A. or related social services education
- Minimum of 4 years of experience working with individuals with disabilities and with adults in community settings
- Ability to transfer knowledge and awareness into behaviors and expectations recognized as appropriate by members of identified cultures and by people with lived experience
- Fluency in American Sign Language (ASL) is considered an asset
- Experience supervising others
- Ability to use the Microsoft Office suite of products, including Word, Excel, Outlook, and SharePoint
- Excellent communication skills including mediating difficult conversations and conflict resolution
- Critical thinking and problem-solving skills
- Excellent time management and organizational skills
The salary range for this 70-hour biweekly position is $26.28 to $33.90 per hour, dependent upon education and/or experience. This position is eligible for a full benefits package (which includes Health and Dental Benefits, Long Term Disability Benefits, and a Pension Plan).
The successful candidate will be subject to Criminal Record, Child Abuse Registry, Prior Contact, and Adult Abuse Registry Checks. A recent Driver’s Abstract and Driver’s Safety Rating must be provided. Access to a vehicle is required.
